FLOWN Apollo 12 instrument panel cue card used during lunar return re-entry, card printed recto and verso with with annotations in black marker to recto, and 7 Velcro patches to verso. October 20/29, 1969. 5 ½ x 8 inches, having a rectangular cutout on the upper left corner. With a type certificate SIGNED by RICHARD GORDON, which states: "Apollo XII | Flown with Conrad, Gordon, and Bean aboard Yankee Clipper to the Moon | November 1969."

Apollo 12's Command Module "Yankee Clipper" returned to Earth on November 24, 1969, landing in the Pacific Ocean at 20:58 UTC.  In the Mission Report, the crew noted: "Entry was normal and was conducted in accordance with the onboard checklist. The only noticeable discrepancy during entry was that, although the planned drogue deployment time was given as 8 minutes 4 seconds after entry [as noted on the upper left corner of the checklist at hand], the actual deployment did not occur until 8 minutes 24 seconds."  "Sea-state conditions were fairly rough, and the landing impact was extremely hard... The 16-mm sequence camera had been placed on its bracket in the right-hand rendezvous window to photograph entry but came loose at impact and contacted the Lunar Module Pilot above the right eye [a slightly concussed Al Bean]. Later inspection of the spacecraft revealed that portions of the heat shield had been knocked loose during impact." (pp. 9-28 to 9-29)
