題跋：
（虞）（《蜀道難》詳文不錄）至順辛未（1331）三月望日，蜀人虞集（款）書於趙伯駒蜀道卷後。
（吳）右明皇幸蜀圖乃千里筆也。展卷觀之，不覺寒威生我肌粟。噫！古人所謂蜀道難，信不誣也！閱此圖者其有蚕叢魚鳧之想云？成化庚子（1480）十月吳寬（款）跋。
（董）趙千里蜀道圖得之李將軍幸蜀圖、趙子昂書太白詩，足稱二妙。董其昌題。
（陳）丁巳（1617）冬月同觀於集雲軒，得未曾有。此卷盖弇州家藏物也。眉公陳繼儒。
