There is a diagonal repair to the shaft around 8cm below the head. There are also a number of diagonal flaws running through the shaft and head.

本品玉質溫潤，頂端雕琢蟠桃枝葉紋，線條遒婉流暢，玉工善運巧色，依形而琢，借自然之筆而作薄意雕，蟠桃清麗婉柔，色澤層次分明，為乾隆一朝清宮造辦處工匠上乘之作。其低浮雕琢，雅緻自然，柄身簡素，末端飾卍字錦紋，質樸淳厚，脫俗出塵，為西王母蟠桃更添幾分逸氣。
蟠桃乃玉如意常見之傳統紋飾，普遍為整體如意造型中之局部裝飾。參見一白玉作例，柄身光素，頂端雕琢仙桃枝葉紋，售於香港佳士得2006年5月30日，編號1598。另有一例，飾雙桃卍字蝙蝠紋，售於紐約佳士得2004年3月24日，編號53。「三多」玉如意亦常以蟠桃作頭，柄身飾佛手，末端雕石榴。另有一作例，柄身同刻御詩，頂端浮雕游龍戲珠，署乾隆癸巳（1773）年款，曾兩度售於香港蘇富比，分別為1987年11月25日，編號370及2010年10月7月，編號2106。
本品紋飾及玉如意本身皆富吉祥祝願，寓意萬事如意，萬壽千秋。如意原為僧侶、仙人所用之搔癢爪扙，後經道家方士引入心形端頂而成靈芝狀。如意作古時爪杖已漸成遺制，後來配以各種紋飾主題，為不同作呈祥獻瑞之物。雍正一朝，以天瑞正王政，重興如意之吉祥喜慶意趣，宮中造辨以不同材質作瑞芝如意工藝。如意常用於內廷錫賚，亦為進獻皇室顯貴以慶壽華誕或晉爵等之美祥賀儀。
