With Patek Philippe Certificate of Origin dated 1996, Extract from the Archives confirming sale on 17 April 1996 and instruction manual.

The production of the Ref. 5004 commenced in 1995 and was the most complicated model in Patek Philippe's line of chronographs, until it was discontinued in 2009.  Composed of 407 miniscule parts, the present example houses calibre CHR 27-70Q, and ingeniously combines a split seconds chronograph and perpetual calendar in a classic 18k pink gold case. The split seconds chronograph allows the simultaneous measurement of multiple events beginning at the same time and employs Patek Philippe's patented spilt seconds disengagement mechanism, accurate within a fraction of a second. The perpetual calendar function features a 24-hour display, leap year indication and precision moon-phases device.
