- Dated October 23, 1967.
- Relating to performances at the Villa Capri in Albany, New York between the 24th and 29th of October, 1967.
- The one-page contract has been signed at the bottom by Peter Criss in blue pen using his legal name Peter Criscuola.
- Accompanied by a copy of a certificate of authenticity which has been signed by Lydia Criss, Peter Criss' first wife.
