1 page (15 x 11 1/4 in.; 380 x 285 mm) on partially printed vellum, accomplished in a secretarial hand, embossed paper seal of the United States with white ribbon. [With] 2 page manuscript inventor's description on 1 sheet of vellum; previously folded, with some discoloration, soiling, and fading, adhesive residue to corners of inventor's description verso.
