Ref 5396's design was inspired by the firm's work in the 1930s. The railway, or sector-style two-tone dial and enlarged Calatrava case are modern interpretations of the Patek Philippe's trademark aesthetic.

Along with the Ref. 5150 and 5125, the Ref. 5396 was among the first modern annual calendar wristwatches to display the day and month in a linear format. Up until this point, annual calendar wristwatches traditionally displayed this information within subsidiary dials and the use of apertures for day and month was reserved for perpetual calendar wristwatches.

In 2010, Patek Philippe replaced the sector dial with traditional gold markers, therefore making this earlier version more sought-after by collectors.
