4to (209 x 154mm.), hand-coloured woodcut printer's device on title-page, printed in red and black throughout, woodcut initials and illustrations, all with contemporary colour, 9 leaves of contemporary manuscript in red and black bound at front (possibly incomplete), contemporary calf gilt over wooden boards with central stamp of the Crucifixion, two metal corner-pieces, two clasps, red edges with paper index tabs, foot of title-page excised (slightly affecting date), lacking two leaves of preliminaries (blanks?) and text leaf t3, small marginal tear on h1 and s6 (from lost index tabs), l1 and o1 repaired along foredge, l2 and n7 torn without loss, x4 torn and crudely repaired without loss, y4-5 and y8 repaired in margins, old reback, lacking straps, lacking other bosses, lower board split and repaired, binding rubbed and scraped
