Alciati fit à Milan une brillante carrière de portraitiste officiel et mondain. On cite de lui le Portrait de S.A.R le Prince de Piémont (1927), un temps exposé au Palais du Sénat à Rome.
Son habileté à évoquer le statut social et l'élégance de ses modèles, dans ses portraits peints avec expressivité et modernisme, proches de Boldini ou de Sargent, lui valurent la clientèle d'une élite cultivée et fortunée.
