The movement is running, the calendar advances when advanced and the correctors all functions correctly when tested. The chronograph functions correctly when tested, on occasion and mostly if the function is not used for a while, the co-axial split-button will stick in the crown but will quickly be released if the crown is rotated, this could be perminantly corrected very easily with lubricant. The dial is in excellent condition as are the hands. The case retains good stamps on the outer shoulders as well as strong definition to the upper edges of the shoulders. The case has no significant scratches or signs of use other than evidence of where the spring-bars have been removed on the underside inner edges of the shoulders, offered in overall excellent condition.

作為嚴謹製錶工藝的極致體現，5004型號可謂鐘錶業的終極時計。型號於1995年開始生產，直至2009年停產。此型號一特別之處是搭載了由Nouvelle Lemania打造的最後一枚非百達翡麗自製計時機芯。最近被大錶盤自製機芯的5204型號取替，5004型號是品牌最具代表性的作品。

由407個零件組成，此錶款搭載了calibre CHR 27-70Q機芯，將追針計時及萬年曆功能完美結合。追針計時實現多項計時同時進行且引進百達翡麗專利的追針脫離裝置，準確率在一秒之內。萬年曆功能還配備了24小時、閏年及精準月相顯示裝置。

此型號曾登載於M. Huber and A. Banbery, Patek Philippe所著《腕錶》一書第二版第二卷312頁上。
